Horizon Spine and Pain Specialists provides patients an unparalleled experience in interventional pain management diagnosis and treatment. Our physicians are board certified in anesthesiology and pain management to provide state-of-the-art interventional treatments. We can often see patients within 24 hours.
We focus on interventional pain procedures and supporting medications with an emphasis on non-opioid therapies to control and relieve pain. Many conditions are easily treated in our office with conservative therapies. For more complex cases, we have the expertise to offer the most advanced pain management techniques available.
Dr. Fischer is a Board Certified Anesthesiologist who specializes in Pain Management. He has practiced in northern Virginia since 1998.
He is the Medical Director of the Horizon Spine and Pain Specialists and is Medical Director of Pain Management Services for the Inova Fairfax Medical Campus. Dr Fischer has been a physician with American Anesthesiology of Virginia since 2007 and he joined the North American Partners in Anesthesia / ProCare Pain Solutions team in May of 2020.
Dr. Fischer focuses on providing pain relief through a multimodal approach that includes non-operative injections/procedures, rehabilitative therapies, medications, adjunctive therapies, and education. Dr. Fischer also has lectured extensively in regard to combatting the opioid epidemic and pain management. He provides frequent CME lectures in regard to proper opioid prescribing for the Virginia Board of Medicine. He is a Co-Chair of the Inova Health System's Opioid Task Force. Dr. Fischer is also the Team Pain Management Physician for the Washington Football Team and an Assistant Professor at the University of Virginia School of Medicine, Inova Fairfax Regional Campus.
Dr. Fischer graduated from the University of Pittsburgh Medical School in 1993. He completed his residency training in anesthesiology at the Brigham and Women's Hospital in 1997 and he was selected as the Harvard Combined Fellow in Pain Management from 1997 - 1998. He is board certified in both Anesthesiology and Pain Management.
Dr. ChenChen Costelloe is a dual Harvard-trained, board certified Anesthesiologist and Interventional Pain Management specialist with Horizon Spine and Pain Specialists. She also serves as an anesthesiologist at the Inova Fairfax Medical Campus.
Dr. ChenChen Costelloe, received her Bachelor of Science degree in Microbiology and Biomedicine from Clemson University in Clemson, South Carolina, where she graduated with honors. She attended medical school at the Medical University of South Carolina in Charleston, South Carolina, where she received her Doctor of Medicine degree. Dr. Costelloe went on to complete her internship and residency in Anesthesiology at Brigham and Women's Hospital/Harvard Medical School where she served as chief resident and graduated with distinctions in Research, Education, Medical Knowledge and Patient Care.
Following her internship and residency, Dr. Costelloe completed her Interventional Pain Management fellowship at Brigham and Women's Hospital/ Harvard Medical School, recognized as the Top Pain Management fellowship in the nation. She was fortunate to train under the top innovators and leaders in the field of Pain Medicine.
Dr. Costelloe believes in a comprehensive and multi-disciplinary approach to pain management, including physical therapy, psychosocial support, and interventional pain management procedures according to the latest and most evidence-based guidelines.
Dr. Costelloe has expertise in a range of treatment modalities including spine, joint, and musculoskeletal injections; regional anesthesia and peripheral nerve blocks; nerve ablations; spinal cord and peripheral nerve stimulation; dorsal root ganglion (DRG) stimulation. Dr. Costelloe has a special interest in neuromodulation, cancer related pain as well as minimally invasive surgical techniques.
In her time away from work, Dr. Costelloe likes to travel, explore new restaurants and play tennis.
The Washingtonian Magazine has named Dr. Greg Fischer as one of the 2022 Top Doctors in Northern Virginia. Dr. Fischer was selected as a top pain management provider in the Washington, D.C., region by his peers. The criteria utilized for this nomination include: internal evaluations, patient reviews, and overall quality of care.
